Package General Terms & Conditions:
    •    STENT/s / PACEMAKER DEVICE/s are not included in the package.
    •    Patients must be admitted to the room indicated in the package. Otherwise, the procedure will be billed on an itemized basis.
    •    For HMO members and Corporate Accounts of De Los Santos Medical Center, coverage of packages is subject to approval prior to availment.
    •    The PhilHealth benefit of the patient will be deducted upon discharge, provided all requirements/documents have been accomplished and presented.
    •    Packages cannot be used simultaneously with any other government-mandated promo or discount such as Senior Citizen or PWD discount.
    •    Patient should sign the Package Availment and Agreement Form upon admission to avail the package.
