Clinical Pharmacist

1. Conducts Drug Therapy assessment to determine whether the prescribed medicines are optimally meeting the patient’s needs and goals of therapy

  • Assess appropriateness and effectiveness of the patient’s medications
  • Checks for the completeness of the doctor’s medication order
  • Recognizes untreated health problems that could improve or resolve with appropriate medication therapy

2. Follows the patient’s progress to determine the effects of the patients medications on his or her health

3. Conducts patient’s counselling in order to inform the patient on how to best take his medications

4. Refers the patient to physician or other health professionals to address any drug therapy problem

5. Applies specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects and drug interactions in performing their patient activities in collaboration with other members of the health care team

6. Calls on their clinical experience to solve health problems through the rational use of medications

7. Documents the clinical pharmacy services provided through the different clinical pharmacy forms

8. Other tasks maybe assigned by the immediate supervisor

 

Job Requirements:

  • Bachelor’s/College Degree , Professional License (Passed Board/Bar/Professional License Exam), Clinical Pharmacy/Pharmacology or equivalent.
  • At least 1 year(s) of working experience in the related field is required for this position
  • Preferably 1-4 Yrs Experienced Employees specializing in Healthcare – Pharmacy
